ABOUT THE SURVEY
Since the coronavirus pandemic, the infection control and prevention measures introduced to mitigate the spread of the virus have led to many changes in everyday life. We would like to learn how these changes have impacted the lives of LGBTQ+ Americans. Wellness with Pride: COVID-19 and LGBTQ+ Communities is the nation-wide project to assess the impact of the COVID-19 pandemic on the social, economic, and health inequities and resources of LGBTQ+ adults 18 years of age and older.
The questionnaire generally takes 10 to 15 minutes to complete. We will ask you about your health, social relationships, economic status, and other life experiences in the questionnaire. Your participation is voluntary.

– You are at least 18 years of age.
– You identify as lesbain, gay, bisexual, trans, queer, non-binary, or sexual or gender diverse.
– You currently live in the United States.
By participating, you will be eligible for a drawing awarding $200 Gift Certificates to 12 randomly selected individuals.
Your responses will be securely stored, assigned an identification number, and kept separate from your name and any other identifying information. Project findings will be summarized, and your name and identifying information will never be used in any materials. When your participation in the project is completed, all identifying information will be destroyed after the records retention period required by state and/or federal law.
